The process:

1) Fill out the JFM Involvement Form.  Please give careful consideration to all details.  

This form is designed to be completed by tabbing through and filling in the spaces relevant to your request.  Do not print the form and fill it out.  Instead, save the form to your computer, fill it out, and save it under a different name (for example: JFMGI  HESS or JFMGI Team).

2) Send the form as an attachment to the Africa Region JESUS Film Ministry coordinator.   The email address, fax number and postal address are shown on the form.
